You have reached this page in an effort to resolve an issue, complaint or problem involving a damage prevention stakeholder company in Maryland. All incidents must have occurred no more than 365 days from the incident or discovery date. Notices of Probable Violation (NPVs) may only be submitted for an incident that occurred or was discovered up to (1) one year from the date of submission. NOTE: All NPV's filed up to March 10, 2021 are subject to the 2010 Title XII statute (the old law). In order for a proposed violation to be subject to the new statute, the incident must have occurred after March 11, 2021.

It is the Authority’s strong recommendation that you first attend the MD/DC Damage Prevention Committee Meeting (DPC).

The DPC is a monthly meeting of damage prevention stakeholders operating in Maryland; you will have the opportunity to establish lines of communication and/or to meet with stakeholder representatives to resolve your issue at the LDPM level.

If you have not been able to resolve your issue through communications at the DPC, please continue to the formal Notice of Probable Violation (NPV) page.

There is a $200.00 non-refundable filing fee associated with each submittal.
  • THERE IS NOW A 2021 STATUTE DROP DOWN MENU AVAILABLE ON THE NPV FORM FOR INCIDENTS OCCURRING AFTER MARCH 11, 2021.
  • If the submitted NPV form is not completed properly; contains incorrect, inconsistent or conflicting information; cites the wrong Title XII subsection violated, the Authority reserves the right to reject the NPV prior to beginning any investigation. The Authority will return the NPV to the complainant for further action.
  • Once an NPV has been successfully submitted, it will be reviewed and presented to the Authority for consideration; at which point a recommendation will be made as to the validity of the claim.
  • Only after receiving the recommendation of the Authority, may either the complainant or the probable violator request a formal hearing before the Authority.
